Consultant Physician In Geriatric Medicine - Older Persons Services Donegal for 18.5 hours per week and Letterkenny University Hospital 18.5 hours, NWCPGM02.
This is an appointment to the HSE West & North West on a Public Only Consultants’ Contract 2023 by the Health Service Executive. The initial commitment for this post will be to Older Persons Services Donegal for 18.5 hours per week and Letterkenny University Hospital 18.5 hours.

The consultant Geriatrician in conjunction with Older Persons Services and the wider consultant Geriatrician Group will develop the Donegal Ambulatory Care Hub supporting older people to remain at home and supporting early discharge from acute hospital.
The Consultant Geriatrician will bring an expert evidence based approach to the care of older people in Donegal and will be integral part of the Integrated Care Programme for Older Persons team whose aim is to deliver multidisciplinary care to frail older people especially in the community setting and support the further development of Integrated Care Programme for Older Persons in this area.
This consultant position will also integrate with the existing consultant staff within the Geriatric Service and the wider Department of General Medicine at Letterkenny University Hospital.
Be a single point of contact for Geriatric Services (Ambulatory Hub Approach) for all stakeholders, including GPs, will provide an interface between acute and community services.
Enable enhancement of community based services such as Reablement/Falls Prevention and Frailty.
Develop and extend the Ambulatory Hub for Donegal in conjunction with existing Donegal Integrated Health Area services and the Operational Lead for Integrated Care Programme for Older Persons.
To co-ordinate and implement the Ambulatory Care Hub Pathway for Older People identified for case management across all service interfaces. Specifically, this post will lead out in providing the specialist input to Community Services in supporting early discharge initiatives/admission avoidance/GP support.
Develop and maintain communication with people on complex matters, issues and ideas and/or in complex situations.
HSE West and Northwest includes all hospital and community healthcare services in the region.
This role is in Donegal Integrated Healthcare Area with a population of 159,227 with 28,130 over 65 years. All acute and community services for Donegal are the responsibility of one Integrated Health Area Manager enabling further development of integrated care.
Letterkenny University Hospital is part of the former Saolta University Healthcare Group and is the second-largest Model 3 hospital in the HSE network serving a population of over 160,000 people in the catchment area of County Donegal, in the Northwest of Ireland. The hospital has seen substantial growth in the last number of years and in 2023 there were over 47,000 ED attendances and over 25,000 hospital admissions.
The appointment is whole-time, permanent and pensionable.
The annual salary will be as set out in the Public Only Consultants’ Contract 2023. Medical Consultants Salary Scalesfrom 1st August 2025:
€233,527 €246,150 €259,475 €266,489 €273,501 €280,513
The standard working week applying to the post is: 37 hours per week
The annual leave associated with the post is: 30 Working Days per annum
